위장관 출혈 (Gastrointestinal bleeding, GIB)

임상양상

토혈(hematemesis)

: ligament of Treitz (소장과 십이지장의 경계) 상부의 upper GI bleeding

• 붉은색 or 커피색 피를 토함

  : vomitus of red blood or “coffee-ground” material

• 객혈(hemoptysis)과의 감별

  • 객혈은 혈액이나 혈액이 포함된 객담을 기침과 함께 배출하는 것을 의미
  • 객혈의 기저질환은 심폐질환
  • 객혈은 거품이 있고, 색은 토혈보다 밝으며 산성인 토혈과 달리 알칼리성
  • 대변 잠혈반응이 음성

흑색변(melena)

: ligament of Treitz 상부의 출혈이 GI tract에서 14시간 이상 있을 경우

• 검고, 타르 같은, 악취가 나는 대변(black, tarry, foul-smelling stool)
• Bismuth, 철분 제제 복용 시에도 흑색변이 나올 수 있음
• transit time이 연장된 경우에는 Treitz 하부라도 melena가 나타날 수 있음 → 변비 환자의 melena는 LGIB일 수 있음
